How often do you have to renew medicaid qmb?

I just renewed my Medicaid QMB in March 2011 but I can't seem to reach anyone at the Medicaid office to ask when I need to fill out a renewal form so I won't have any gaps in coverage. Please help me if you know anything about Medicaid QMB...thanks
